The patient lived in Greensboro, but could not him to get myocardial biopsy done in Greensboro..
The diagnosis was amlyodosis. There was severe biatrial enlargement suggestive of restrictive cardiomyopathy in the absence of sever MR and TR . Mitral inflow pattern showed severe restrictive filling of the LV, high velocity jetand extremely short deceleration time . Hypotension is also extremely common in amyloidosis,
The treatment has significantly changed since 2003!
